PROCESSING OF PERSONAL DATA RELATED INFORMATION FORM
- Obtaining, Processing and Purposes of Processing Personal Data:
Your personal data is collected and obtained verbally, in writing or in visual or electronic media through the call center, website, verbal, written or other similar channels in order to carry out services such as the protection of public health, preventive medicine, medical diagnosis, treatment and care services, planning and management of health services and financing offered by One Dose Health and in accordance with the business areas of One Dose Health.
Your general personal data, especially your health data, and your special categories of personal data may be processed by One Dose Health for all kinds of purposes and purposes, including but not limited to the following purposes:
- Your identity data and information: Your name, surname, TR Identification Number, passport number or temporary TR Identification Number, date and place of birth, marital status, gender, social security or patient protocol number, nationality and ethnicity, and other identification data that may help us identify you; and
- Your contact data: Your address, telephone number, e-mail address and other contact data, and your verbal conversation or call records maintained by customer service or patient care departments according to call center standards, and your personal data collected when you communicate with us by e-mail, letter or other means of communication; and
- Your accounting data: Your bank account number, IBAN number, credit card data, billing data and other similar financial data and information; and
- Your private health insurance data and Social Security Institution data and information required for the financing and planning of health services; and
- Your health information: Your personal data relating to your health and sexual life obtained and collected during the provision of services or as a result of medical diagnosis, treatment and care services, including but not limited to your laboratory analysis results, test results, medical examination data, appointment data and information, check-up data and prescription data and information.
Any of your personal data (including but not limited to your sensitive personal data) obtained by One Dose Health may be processed for the following purposes
- For confirmation of your identity and
- Protection of public health, preventive medicine, medical diagnosis, treatment and care services, planning and management of health services and financing,
- Measurement, improvement and examination of the patient and
- If you make an appointment, we will inform you about the appointment and
- Fulfillment of risk management and quality improvement activities,
- Conducting analyses for the further development of health services and
- Your healthcare services are financed by the Patient Services, Financial Affairs and Marketing departments and your medical examination, diagnosis and treatment costs are paid as part of your eligibility inquiry and the requested information is shared with private health insurance companies; and
- Conducting research and studies and
- Compliance with relevant legal and regulatory requirements
- Sharing requested information with private health insurance companies during the financing of health services, and
- Conducting risk management and quality improvement activities by the Quality, Patient Experience and Information Systems departments; and
- Preparation of invoices by Patient Services, Financial Affairs and related departments for our services and confirmation of your relations with contracted institutions and companies,
- Media & Communications and Call Center departments design and communicate campaign participation and campaign information, special content and discrete and intangible benefits across web and mobile channels.
Your personal data obtained and processed in accordance with applicable laws and regulations may be transferred to One Dose Health’s physical archives and/or information systems and stored and maintained on both digital and physical platforms.
- Transfer of Personal Data
Your personal data may be shared with private insurance companies, the Ministry of Health and its sub-units, the Social Security Institution, the General Directorate of Security and other security forces, courts and all other judicial authorities, the center, etc.
with your consent and limited to your consent.
One Dose Health may share your personal data with third parties, including your authorized representatives and agents and third parties providing consulting and advisory services to us, including but not limited to lawyers, tax and financial advisors and auditors, and our business partners and other third parties acting in collaboration with us for the further development or fulfillment of healthcare services for the purposes set out above, as well as regulatory and supervisory bodies and authorities and governmental authorities, in accordance with the GDPR and other applicable laws and regulations for the processing purposes listed above.
- Methods and Legal Grounds for Collection of Personal Data
Your personal data is collected and processed in all kinds of verbal, written, visual or electronic media for the purposes listed above and for the performance of all kinds of work within the business areas of One Dose Health within the legal framework and accordingly for the full and proper performance of all contractual and legal duties and obligations of One Dose Health.
The legal reasons for collecting and obtaining your personal data are the relevant provisions of the following:
- Law No. 6698 on the Protection of Personal Data,
- General Data Protection Regulation No. 2016/679 (“GDPR”) and Law No. 3359 on Primary Health Care Services and
- Decree Law No. 663 on the Organization and Duties of the Ministry of Health and its Affiliated Companies and
- Regulation on Private Hospitals,
- Regulation on the Processing and Protection of Privacy of Personal Health Information and
- Ministry of Health regulations and other legislative provisions.
In addition, personal data relating to health may also be processed by persons under the obligation of confidentiality or authorized official institutions and organizations, without being obliged to obtain the prior explicit consent of the relevant purpose, only for the protection of public health, preventive medicine, medical diagnosis, treatment and care services, and the planning and management of health services and financing.
- Your Rights on Protection of Personal Data:
Under the Law and GDPR and other applicable laws and regulations, you have the following rights:
- To learn whether your personal data is being processed,
- Requesting information if your personal data has been processed and
- Access and request personal health data and information, and
- To learn the purpose of processing my personal data and whether they are used for their intended purpose,
- To know the third parties resident in Turkey or abroad to whom your personal data are transferred, and
- To request correction of your personal data in case of incomplete or incorrect processing,
- Request the erasure or destruction of your personal data,
- In case your personal data is incomplete or incorrectly processed, to request correction or completion of these and/or deletion or destruction of your personal data to be notified to third parties to whom your personal data has been transferred,
- To object to the emergence of a result to the detriment of the person himself/herself by analyzing your processed personal data exclusively through automated systems,
If you exercise any one or more of your rights set out above at any time, the relevant information will be communicated to you in writing or electronically in a clear and understandable manner using the contact data and information provided by you.

One Dose Health protects your personal data by fully and strictly complying with all technical and administrative security controls that must be taken in accordance with information security standards and procedures.
These security measures and precautions are taken and provided at a level appropriate to the possible risks, taking into account technological possibilities.

Your personal data is protected within the framework of the existing technical and administrative possibilities, necessary security measures and precautions are taken by taking into account the technological possibilities and are provided at a level appropriate to possible risks.

- (iii)[info@onedose.io] via secure electronic means or mobile signature, via your registered e-mail address or your electronic e-mail address registered in our system and/or
- (iv) by signing a file in “word or pdf”.
